Qualifications:
Holds a Masters Degree and:
1. The certificate of Clinical Competency of the American Speech-Language-Hearing Association, and state licensure as a licensed speech
pathologist, has at least nine months of experience, OR
2. Meets the Requirements of the American Speech-Language-Hearing Association for Clinical Fellowship Year (CFY)
Responsibilities:
1. Performs in accordance with applicable standards and requirements imposed by federal, state and local laws, rules and regulations including the
Professional Code of Ethics and Standards of Practice of the Profession.
2. Screens patients to determine if new admissions or current residents would benefit from skilled services.
3. Evaluates patients to determine baseline status, strengths and weaknesses in speech/language, swallowing or hearing
4. Develops and implements treatment plans for patients to restore and maintain their highest level of functioning, and reassess treatment results.
5. Participates in discharge planning.
6. Completes all written documentation in patient's chart in a clear concise manner, including daily, weekly, and 14 day progress notes, monthly
and discharge summaries and care plans, on the appropriate forms as it relates to reimbursement guidelines and the SPRS policies and procedures
7. Communicates regularly with the patients physician, family and members of interdisciplinary team regarding patient's progress, plan of care and
discharge plans.
8. Attends rehabilitation conferences, utilization review meetings, care-plan meetings, weight committee meetings etc. as requested by the facility and
approved by SPRS corporate office.
9. Develops programs in the facility that will aid in the care of the patients.
10. Provides in-service training to staff in facility regarding services provided by the speech pathologist and strategies to improve speech/language and
swallowing for specific patients of group of patients.
11. Follows relevant physician orders for evaluation and treatment.
12. CFY Works under the supervision of a certified Speech-Language-Hearing Pathologist for a specified period of time as per the requirements of the
American-Speech-Language Association and State Licensure
13. Carries out assignments as given by the Supervising Speech Pathologist, Rehab Coordinator, Area Manager and/or Director.
14. Follows administrative policies and procedures related to emergency procedures, infection control and safety.
