Qualifications:

  California Registered Physical Therapist with a minimum of one year experience.

Responsibilities

    1. Performs in accordance with applicable standards and requirements imposed by federal, state and local laws, rules and regulations including the

        Professional Code of Ethics and Standards of Practice of the Profession.

    2. Screens patients to determine if new admissions or current residents would benefit from skilled services.

    3. Evaluates Patients to determine baseline status, strengths and weaknesses.

    4. Develops and implements treatment plans for patients to restore and maintain their highest level of functioning, and reassess treatment results.

    5. Carries out assignments, as given by the Supervising Physical Therapist

    6. Makes a full report to the Supervising Physical Therapist of any treatment given, any changes in orders, or new orders received.

    7. Instructs and supervises assistants assigned to him/her.

    8. Participates in discharge planning

    9. Completes all written documentation in patient's chart including daily, weekly, and 14 day progress notes, monthly and discharge summaries and care-plans.

  10. Communicates regularly with the patients physician, family and members of interdisciplinary team regarding patient's progress, plan of care and

        discharge plans.

   11. Attends rehabilitation conferences, utilization review meetings, care-plan meetings, weight committee meetings etc. as requested by the facility and

        approved by SPRS corporate office.

   12. Develops programs in the facility that will aid in the care of the patients.

   13. Provides in-service training to staff in facility regarding services provided by the occupational therapist and strategies  to improve activities of daily living

         for specific patients or groups of patients.

    14. Follows relevant physician orders for evaluation and treatment.

    15. Follows administrative policies and procedures related to emergency procedures, infection control and safety.

 Physical and Specific Qualifications:

    1. ability to handle intense physical demands of lifting, handling and moving patients, moving through the facility, stooping and working at different heights.

    2. ability to communicate effectively with all personnel and patients.
